It depends on what the minutes is being prepared for.
Generally speaking (though there are variations depending on what the minutes is being prepared for):
It is usually on the letter headed paper having a title (e.g. Extracts of Minutes of Meeting of (name of the trust)). Then you put the place and date of convening.
You list the members present, state matters arising, state the other business(es) transacted at the Meeting and then you state the adjournment date for the next meeting.
The minutes should be signed by the Chairman and Secretary of the NGO.
